Lost or Stolen computer / device

What is it?

This form is intended to collect as much information as possible about lost devices to help identify the nature of any data that may be exposed, and to assist in the recovery of the physical device. Please fill out all fields with as much detail as possible to assist in our investigation. Speed is of the essence, so please submit this form as soon as possible after the loss. Who can request this?

Any MSU Denver student or employee who has an MSU Denver device that was lost or stolen should report that here. 


We want you to report the basics of the loss as soon as possible.  As we continue to work with you we will need a copy of the police report filed on this loss/theft. We can help you find the device's asset tag and serial number if you don't have that to file your report. 

Be prepared to share what kind of data was stored on the device (e.g. Personally-Identifiable Information, FERPA data, etc.) so we can assess what type of data may be exposed. 

What to expect

ITS will email you back from this ticket and/or call you to help ensure you are able to complete a police report and upload that to the ticket.  Our teams can help you acquire a loaner device in this process.
