Employee Separation

What is it?

Human Resources and IT Services partner to properly off-board and remove access when employees separate. 

Who can request this?

Supervisors with a separating employee will be required to use this form to start the off-boarding process for the employee's accounts.


Supervisors will be required to provide the separating employee's NetID, 90 number, separating date and time, and for full-time employees, a letter of resignation.

What to expect

This form will inform Human Resources of the impending separation to begin the offboarding process. Once Human Resources approves the separation, Information Technology Services will receive a ticket that will schedule account deletion and device pickup for the date and time noted by the supervisor.
